NDIS – Booking an Initial Assessment
To book an assessment at the Coburg North clinic, you must contact them directly via phone or email.
- Phone: 0432 444 756
- Email: info@perfectfitpedorthics.com.au
- Referrals: While they work with various funding schemes, a written referral from a medical specialist or allied health professional is typically required for services.
What to Expect During the Assessment
The assessment is a comprehensive clinical process focusing on your mobility and footwear needs:
- Initial Consultation: Gathering medical history and daily activity details.
- Clinical Analysis: Includes gait analysis (watching how you walk) and foot analysis to identify biomechanical imbalances.
- Footwear Review: Examination of your current shoes to see if they provide adequate support or contribute to pain.
- Treatment Plan: Development of goals, which may include custom orthotics, footwear modifications, or medical-grade shoes.
NDIS Registration & Funding
As a registered NDIS provider, they can supply products and services to participants whose funds are managed by the NDIA, as well as those who are self-managed or plan-managed.
- Specialist Reports: They provide Assistive Technology (AT) Reports or Outcomes Reports to support funding claims under “Customised Prosthetics and Orthotics” (Category 0135).
- Reasonable and Necessary: Their reports help demonstrate that specialized footwear is directly related to your disability and will increase your independence.
- Service Agreements: An initial consultation is used to complete a Service Agreement before progressing to formal AT assessments and reports.
The NDIS Service Journey
Regardless of how your funds are managed, Perfect Fit Pedorthics follows this clinical process to ensure your footwear or orthotics are funded:
- Initial Consultation: Gathering medical history and contact details.
- Service Agreement: A mandatory step to formalise expectations before progressing to an assessment.
- AT Assessment & Quote: They create an Assistive Technology (AT) report and quote, which includes evidence of why the support is “reasonable and necessary” for your disability.
- Approval & Treatment: Once the NDIS approves the quote, they commence the treatment plan, including fitting and any necessary modifications.
Essential Next Steps
- Check your budget: Ensure you have enough funding in your Core (for low-cost items) or Capital (for high-cost AT) budgets.
- Contact the clinic: Reach out to info@perfectfitpedorthics.com.au to request a copy of their Service Agreement to review before your first visit.
Documents to Bring
- NDIS Plan & Details: Your NDIS number and a copy of your current plan (specifically your Core and Capital budgets).
- Contact Info: Your Support Coordinator’s or Plan Manager’s name and email.
- Current Footwear: Bring the shoes you wear most often, as the wear patterns on the soles give them vital clues about your gait.
- Existing Orthotics: Any old or current inserts you are using.
- Medical Reports: Recent scans (X-rays, MRIs) or letters from your Podiatrist or GP regarding your feet or mobility.
